Delusional Thomas is one of several alter egos created by Pittsburgh rapper Mac Miller, distinguished by a deliberately pitched-up, cartoonish voice and dark, horrorcore-inspired lyrical content. Mac described the character as a psychopathic figure, a stark contrast to the more introspective and melodic persona he presented under his own name.

The voice first appeared, initially uncredited, on the intro track of Mac's 2013 album Watching Movies with the Sound Off, setting an unsettling tone before the record proper began. The character was later fleshed out into a full mixtape released under the Delusional Thomas name, featuring tracks such as «Bill», «Larry», and «Grandpa Used to Carry a Flask», which lean into surreal, violent, and absurdist imagery delivered with dark comic energy.
